ICD3 on with PIC32MX250F128B

Has anyone connected the ICD3 (using MPLAB X) and seen funny results? My debugger crashes a lot and I keep having to reset the debugger to get it working again. Often the debugger gets lost in the program and cannot debug after programming. It just halts before the first program line.
